Legal Documentation and Evaluation FAQ’s

Does WellQor provide paperwork for legal cases, disability claims, FMLA, or workers’ compensation?

Generally, no. WellQor’s therapists focus on clinical evaluation and psychotherapy to help improve your mental health and daily functioning. Because of this, we do not complete documentation intended for:

    • Disability applications or appeals
    • Legal cases involving emotional harm
    • Divorce or custody disputes
    • Court-ordered evaluations or statements
    • Fitness-for-duty or return-to-work requirements
    • Any paperwork designed to influence a legal or administrative decision

Clients who need documentation for these purposes may be better served by a provider who specializes in forensic or evaluation-based assessments.


What documentation can WellQor provide?

We maintain standard clinical documentation as part of your care. This includes diagnosis, treatment, and progress notes.

We may consider additional documentation (such as treatment verification, letters of support, recommendations, or a brief summary) only when:

    • You have completed at least 12 weeks of therapy.
    • You’ve had around 10 or more sessions.
    • There has been meaningful therapeutic participation and progress.

Even then, these requests are not guaranteed and are approved based on clinical appropriateness at the discretion of your therapist and the Clinical Director.


Is there a fee for additional documentation?

Yes. Any documentation outside of standard clinical notes requires dedicated clinician time and is not covered by insurance. Additional paperwork is billed at $200 per hour, which includes preparation, review, and coordination.


Why doesn’t WellQor complete legal or disability-related paperwork?

Our mission is to support your mental health by helping you build coping skills, reduce symptoms, and improve everyday functioning.

Therapy is most effective when it is focused on personal wellbeing, not on meeting legal or administrative requirements. When documentation is needed for those purposes, it requires specialized evaluations that fall outside the scope of routine clinical care.


What if I need documentation for a legal or administrative matter?

If your primary goal is to obtain documentation for a legal case, disability claim, custody dispute, or similar process, WellQor will not be a good fit for your needs. You should seek a provider who specializes in those types of assessments. They are best equipped to meet the requirements of those situations.
